The Certification Bureau is responsible for ensuring that Montana facilities accepting Medicare and Medicaid payments meet the federal regulations required by the Centers for Medicare and Medicaid Services (CMS). The Certification Bureau surveys the following facility types: hospitals, nursing homes, ambulatory surgical centers, rural health clinics, home health agencies, hospices, end stage renal disease facilities, psychiatric residential treatment facilities, outpatient physical therapy providers and intermediate care facilities for the mentally retarded.
The Health Inspection Surveyors look at many aspects of a facility's operation, including the care of residents and clients, the manner in which staff and residents or clients interact, and the environment
The Life Safety Code (LSC) Surveyors evaluate whether a facility meets standards set by the National Fire Protection Agency (NFPA). This survey covers a wide range of fire protection, construction, and operational features
This webpage will show survey results for most of the surveys that the Certification Bureau performs. This site only has surveys from March 1, 2011 forward. If you want to view a survey prior to this date, please submit your request to mtssad@mt.gov or 406-444-2099.
The Statement of Deficiencies (SOD) is the findings of the Certification Bureau after the survey. Long term care "Statement of Deficiencies" will be released on the 15 th day after the document is available to the facility, which is 10-15 days after the survey. All other facility "Statement of Deficiencies" are released 90 days after the survey is completed.
The Plan of Correction (POC) includes the Statement of Deficiencies (SOD) as well as the facility's response on how it plans to correct the deficiencies. The POC cannot be released until it has been reviewed and accepted by the Bureau, therefore the time frame in not automatically set
Surveys are searchable by facility name or city location.
